He was predeceased by: his father David W. (Marcella M.); and his brothers, Robert and Gary Cook. He is survived by: his wife Molla Cook; his daughters, Kati Lin Cook (Jessica Pierce) and Jennalee Marie Cook (Michael Wagner); his sister Karla Zike (Kevin); and his sister-in-law Barb Cook. He is also survived by nieces and nephews.
