He was predeceased by: his parents, Frank Lewis and Wanda Duggins Lewis; his brother Frankie'Chuck'Lewis; and his stepsister Janice. He is survived by: his wife Starla Lewis; his son Tony Lewis of Baylis, IL; his grandson Dana Lewis (Justine) of Baylis, IL; his great grandchildren, Ross Lewis and Preston Lewis; his siblings, Tom Lewis (Mandy) and Cynthia Forshey (David); and his stepsister Dixie Alvies. He is also survived by numerous nieces, nephews, great nieces, and great nephews.
