He was predeceased by: his parents, James Ernest and Shirley Mae Van Hook (Wittekiend); his stepfather Ladell Tittsworth; his grandparents, James, Ruth Van Hook, John and Josephine Wittekiend; his uncles, William and Harold Van Hook; and his nephew James Allen Van Hook.
He is survived by: his siblings, Ernie Van Hook, Cory Van Hook (Connie) and Melissa Coultas (Randy) of Barry, Illinois.; and his nieces and nephews, Theresa, Lora, Julie, Jennifer, Kelly, Chris and Shelly.
