He is survived by: his former wife Jill Beckstrom; his children, Ryan Slater of Slater and Ashley Jorgensen (Jeremy) of Coal Valley, Illinois; his grandchildren, Lance, Avery Jorgensen of Coal Valley, Cam and Annah Mozingo of Slater; and his siblings, Jason Slater (Jen) of Cordova, Kelli Larkin (Joe) of Rock Falls, Jeremy Slater (Lienda) of Bartlett, Brian Jennings (Lanette) of Avon, Laura Jennings of Avon, Brenna, Briar Jennings of Avon and Jake Slater of Rock Falls.
Memorial contributions may be made to McDonough District Hospital Hospice and left in the care of the funeral home.
