She was predeceased by: her parents, Forrest Johnson and Hazel Johnson (Hoover); her spouse Charlie E. Fowlks; her grandson Nick; and her son-in-law Rob Mills.
She is survived by: her children, Todd Fowlks (Jill) of Bushnell, Patricia Mills of Delavan, WI, Charles Fowlks of Avon and Barbara Pool of Avon. She is also survived by 3 grandchildren.
