He was predeceased by: his parents, Alfred Boelens and Viola Boelens (DeSutter); his wife Patricia Yates; his son Mark Boelens; and his brother Dick Boelens.
He is survived by: his children, Bryan Boelens of Sioux Falls, SD, Cathy Hall of Mineral, IL, Gwen Shaw of Long Island, NY and Paula Shaw of Iowa City, IA. He is also survived by 3 grandchildren and 3 great grandchildren.
Memorials can be made in his name to the American Alzheimer's Association or the American Kidney Foundation.
