He was predeceased by: his parents, Jack Forth and Carolyn Huffman (Chandler); his stepfather Robert Huffman; and his brother Donald Forth. He is survived by: his wife Denise L. Forth of Ashmore, IL; his children, Jacob R. Neff (Matthew) of Cape Coral, FL, Nicholas R. Forth of Ashmore, IL and Mackenzie L. Forth of Ashmore, IL; and his siblings, Jenny Stephenson of Ashmore, IL and Bruce Forth (Barbra) of Indianapolis, IN. He is also survived by several nieces and nephews.
Visitation was held on Saturday, January 4th 2025 from 1:00 PM to 2:00 PM at the Adams Funeral Chapel (2330 Shawnee Dr, Charleston, IL 61920). A funeral service was held on Saturday, January 4th 2025 at 2:00 PM at the same location.
