He was predeceased by: his spouse Theresa Niedzwiecki; and his siblings, Vito, Rocco, Patrick, Marie, Madelyn, Angeline and Lucenia.
He is survived by: his children, Margaret Ann DeWolfe of Charleston, SC, Ralph Jacobazzi of Des Plaines, IL and Thomas Jacobazzi (Katie) of Glenview, IL; his grandchildren, Robert, Nicholas Jacobazzi and Christopher Clark (Kelly); and his close friend Marilyn Lashcinski of Indian Creek, IL. He is also survived by many other relatives and friends.
