He was predeceased by: his grandparents, Ted and Awanda Marth. He is survived by: his parents, Donald W. Ashby and Julie A. Marth of Worden, IL; his brother Ben; his sister-in-law Jennifer of Holiday Shores, IL; and his niece Hadleigh Ashby. He is also survived by aunts, uncles, cousin and friends.

Memorials may be made to Autism Speaks. Sam's gift of life was extended through organ and tissue donation through Mid America Transplant Services.
