He was predeceased by: his parents, Robert L. and Henrietta Hoffman Blaise (Hank); and his brother Rodney Blaise. He is survived by: his spouse Jacqueline "Jackie" Hopper; his daughters, Talitha "Mandy" Blaise of Alexander, Megan McCaherty (Patrick) of Jacksonville, Morgan Blaise of Franklin and Olivia Lockman of Peoria; his grandchildren, Kylie, Mathew, Evan, Emma and Thomas; and his siblings, Jan Nergenah (Silvan) of Jacksonville, Laura Davidson (Malcolm) of Jacksonville and Bob Blaise (Julie) of Springfield.
