He is survived by: his children, Kevin (Annie), Ronald, Terry (Amy), Richie, Tom and Linda Slick (John); his grandchildren, Keith, Kraig, Billy, Trace, Tanner, Colin, Henry, Adriana, Alexa and Ava; his great grandchildren, Tyler Rose, Kylie and Abby; and his sibling Don Koziol.
