He is survived by: his mother Janet Eileen Marler Phelps of Idaho Falls, ID; his former spouses, Christine Goodfellow and Courtney Averett; his children, Nathaniel Cary Phelps, CaryAnna McKenzie Averet, Zachary Averett Phelps (Teyler), Tristan John Phelps, Katelynn Jaime Phelps, Micah Averett Phelps (Finn), Riley Ashlynn Phelps and Payton Scott Phelps; and his siblings, Joel Hawley Phelps, Jr. of Idaho Falls, ID, Wendy Eileen Koplin, Rebecca Diane Fitts (Eric) of Pleasant Grove, UT and Christopher Scott Phelps (Sheila) of Idaho Falls, ID.
In lieu of flowers, the family suggests donations to suicide prevention advocates.
