He was predeceased by: his parents, Alex Slivkoff and Anna Karnoff. He is survived by: his daughter Deborah Scarlett; his brother Timothy J. Slivkoff (Teri L. Slivkoff) of Anderson, IN; and his grandchildren, Kylee, Alexander and Rick.

In honor of John, the family asks attendees to bring their used eyeglasses to donate to the local Lions Club.
