He was predeceased by: his parents, Willard Smith and Elizabeth Lakey Poulsen; and his wife Nelly Lou Kelly. He is survived by: his sons, Jerry (Mary Poulsen), Dr. James Kelly (Becky Poulsen) and Terry (Teresa Poulsen). He is also survived by six grandchildren and nine great-grandchildren.
