He was predeceased by: his parents, David Haslam and Myrna Haslam; his son JD Haslam; and his sister Marlene Fellows. He is survived by: his wife Kathy Perry; his children, Tammy Martinez (Juan) and Brian Haslam (Jerilyn); his daughter-in-law Buffie Haslam Hanks (Matt); and his brothers, Lloyd Haslam (Delores) and Robert Haslam (Shirley).
