He is survived by: his wife Susan Cook of Pocatello; his daughter Gina Marie Miller (Richard) of Pocatello; his siblings, Debra Christman (Kenneth) of Fair Oaks and Dale Cook of San Jose; and his grandchildren, Jessica (David), Kaitlyn (Wade), Matthew (Mariah), Jared, Justin, Colby (Hannah), Timothy, Nicolas and Summer (John).
