He is survived by: his mother Sharon Dvorak (Bickel) of Pasco; his wife Shawna Dvorak (Chambers); his sons, Jared Dvorak of Spokane, James Dvorak (Bubba) of Lewiston and Lance Dvorak of Boise; and his brothers, George Dvorak (Peggy) of Pasco and William Dvorak (Josie) of Pasco.
