He was predeceased by: his parents, Verl Thomas Hawk and Harold David Hawk; and his siblings, Rhita Hackworth, Harold Hawk, Larry Hawk and Dennis Hawk.
He is survived by: his wife Jo Ann Goin Hawk; his children, Shawn Thomas Hawk of Vernal, Utah and Crystal Leigh Hawk of Savanah, Georgia; his grandchildren, Raeven JC Spendlove and Gabriel Beau Spendlove of Vernal, Utah; his great grandchild Blakeleigh Rowen Betts; and his brothers, Steve Hawk (Jeannene) and Scott Hawk (Candee). He is also survived by nieces, nephews and cousins.
