He was predeceased by: his parents, Joe Mayeda and Janiel Mayeda; his brother Randy Mayeda; and his sister-in-law Lana Mayeda. He is survived by: his siblings, Linda Mayeda, Greg Mayeda and Corey Mayeda (Rachel); his nieces and nephews, Joe Merrick, Marci Meng (Jakob), Matt Mayeda (Kelsey), Abby, Hannah and Ethan; and his great-niece Sierra Merrick.
At Keven's request, in lieu of flowers he asked that donations be made to the Skyline High School Booster Club (1767 Blue Sky Dr. Idaho Falls, ID) or the University of Utah Kidney and Liver Center (50 Medical Drive E24 Salt Lake City, UT 84132).
