He was predeceased by: his mother Nell Johnson. He is survived by: his children, Heather Mulligan and Jake Folkestad; his sisters, Vicki Briscoe (Scott), Sandy Forman (Jim) and Carol Parker; and his stepfather Don Johnson. He is also survived by many nieces and nephews.
