She was predeceased by: her parents, Vic Cook and Lorreta Cook; and her siblings, Tom, Jimmy and Sally.

She is survived by: her husband Jim Hawkins; her children, Jim Jr., Clay, Patti, David (Kelli) and Ron (Marie); her brother Gary (Jill); her siblings-in-law, Dina Cook and Jim Schreiber; and her grandchildren, Abi, Janika, Hailey, Madison, Travis, Katie, Hannah and Brodie. She is also survived by several nieces and nephews.
