He was predeceased by: his brother Phillip. He is survived by: his parents, Kay Madsen and LaRue Johnson Crook; and his siblings, Carolyn Ferguson (Phil) and Kevin (Denise). He is also survived by six nieces and nephews, and twelve grand nieces and nephews.
