He was predeceased by: his parents, Francis "Franz" Cook and Marita Cook (Wulfekuhle); his parents-in-law, Earl and Rita Kintzle (Naumann); and his brother Dennis Cook.

He is survived by: his wife Ann Kintzle; his children, Ron Cook (Keri) of Manchester, Randy Cook (Amy) of Worthington, Jodi Helle (Dan) of Worthington and Jill Gogel (Scott) of Worthington; his grandchildren, Hayley, Colin, Hunter Cook, Ryan, Maci Cook, Alexis (Cody Baumler), Jordan, Jack Helle, Rebekah, Cole and Lauren Gogel; his siblings, Pat Cook (Mary Fox) of Holy Cross, Duane Cook (Deb Wierck) of Holy Cross, Joan Wilwert (Dan) of Holy Cross, Joyce Lehmann (John) of Bankston and Julie Barton (Rick) of Asbury; his parents-in-law, Arlene Beringer (Dave) of Sherrill, Vince Kintzle (Vickie) of Dubuque, Ken Kintzle of Balltown, Jim Kintzle (Debbie) of Balltown, Sue Hefel (Fritz) of Buenie, Kevin Kintzle (Kim Johnston) of Millville and Linda Coyle (Tom) of Epworth; his extended family, Ronnie and Sherri Wolf; and also, Josh, Kaci and Ryan of Bankston.
