He is survived by: his significant other Lydia Borntrager of Williamson; his children, Annie Innis (Edwin) of Phoenix, Dion, Shyenne, Diontre, Justus and Ayrez; his great grandchild Khizar; his son Kollin Self of Pella; his sisters, JoAnn Roe (Robert), Linda Judd (Rick), Wendy Edwards and Connie Graham (Don); his uncle Junior Self; and also, Rachelle Self of Des Moines, Brittney Ammons (Ryan) of Albia, Berkley, John Self (Ashley) of Knoxville, Noah, Grace, Courtney Marshall of Pella and Jessica Self of Pella. He is also survived by nephews, a niece and cousins.
