He is survived by: his wife Shannon Forke of Whiting, Iowa; his children, Nichole Schafer (Shawn), Brian Forke, Amanda Valencia (Myles), Makayla Carpenter and Michael Utterback (Haylee); his pets, Zoey and Zeus; his grandchildren, Lucas Leinen (Sophia), Tyler Schafer, Ayden Schafer, Sylver McCall, Landon McCall, Samuel Valencia, Kennedy Valencia, Ellie Valencia, Brady Forke, Marshall Utterback and Westyn Utterback; his siblings, Veda Stougard (Norman), Joan Anderson (Bill), Lynn Forke (Sandy) and Gary Forke (Kathy); his parents-in-law, Mike and Sharon Carpenter; and also, Amanda, Makayla and Michael. He is also survived by fourteen nieces and nephews; a lot of great nieces and nephews; and cousins, other relatives, and friends.
