He is survived by: his children, Ryan Jay Cory Lee (Keri), Holly Ann From (Patrick Grassley) and Hollis; his wife Ann; his son Cory Van Den (Lyndsay); and his grandchildren, Ryleigh, Kenzi, Baylen, Ryker, Rhett and Lexi.
