We are sad to announce that on January 7, 2021, at the age of 84, Donna (Diane) Formaro of West Des Moines, Iowa, born in Des Moines, Iowa passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, Junior Clemens and Marian Clemens (Wright); her husband Phillip; and her siblings, Marlene and Bill.
She is survived by: her granddaughter Makayla; her daughters, Michelle Chambers (Mike) and Teresa Terry (Shawn); her grandchildren, Dawn, Schauna and Michael; and her sister Marilyn Hall-Cook (Bruce). She is also survived by several nieces and nephews.
