He was predeceased by: his father Ellsworth Scott Revely; and his grandparents, Art and Madeline Banwart. He is survived by: his sons, Elijah, Gabriel and Isaiah; and his mother Patricia Banwart (Kris Banwart). He is also survived by half- brothers and half sisters, several aunts, uncles, cousins.
