He was predeceased by: his parents, James and Margaret Worthen, Sr. (Hornung); his grandson Tyler; and his brothers, Bob Worthen and Donnie Worthen.
He is survived by: his wife Cynthia Gordon; his children, Mike Worthen (Sheri) of Webster City, Matt Worthen (Sharon) of Norwalk, Iowa and Misty Worthen of Webster City; his grandchildren, Shantell Sickler (Austin), Shyann Moore (Desman), Shania Worthen, Sierra Sinclair, Jenna Worthen, Jacob Worthen, Dustin Worthen, JorDan DeGraw and Hidie DeGraw; his brothers, Jim Worthen (Anna) of Webster City, IA and Troy Worthen (Jennifer) of Fort Dodge, IA; and his friends, Sharon and Arvin Burkhurt.
