She was predeceased by: her parents, Royal E. Cook and Annette Cook (Griggs); her husband Jay; her son Timothy Ray Underwood; her grandson Joshua Knebel; and her brothers, Ray Cook and Doug Cook.
She is survived by: her daughter Jaylynn Bolin (Don) of Cedar Falls; her granddaughter Katelyn Knebel (Axel Wrage); her great grandchildren, Khloe Kramer, Kyler Kramer, Millie Wrage and Audriana Knebel; and her sister Romona McCall (Sylvia Allensworth).
