He was predeceased by: his parents, Robert Prochaska and Eslie Runyan Prochaska; his wife Mildred "Evelyn" Schackow; his son Dan Prochaska; his daughter-in-law Mary; and his siblings, Marion, Eugene, Donald Prochaska, Lois Schwartz and Thelma Moore.
He is survived by: his children, Lyle of Jesup, John Prochaska of Waterloo and Roxzan Bamford (DJ) of Waterloo. He is also survived by six grandchildren, and seven great-grandchildren.
