He is survived by: his former wife Donna Hayes; his daughters, Amy Stephenson (Todd) of Cresco, IA and Carrie Zweibohmer (David) of Denver, IA; his grandchildren, Dustin Meirick, Holly Meirick and Hayley Zweibohmer; his close friend Mary Jo Jordan; and his siblings, Francis "Jake" Jacoby (Myrna) of Waterloo, IA, James Jacoby (Shirley) of Evansdale, IA, Sandy Quinn (Bob) of Glenville, MN and Mary Jean Cartney of Rochester, MN. He is also survived by many nieces and nephews.
