He is survived by: his wife Doretta; his children, Tres Neil of Cedar Rapids, Manina Jackson (David) of Austell, GA, Nisa Neil of Waterloo and Cherie Neil (Matthew O'Connor) of Hudson; his grandchildren, Ella, Taylor Neil, LaTorya Dial, Lakena Mallory-Neil, Bryson Mallory and Brian Mallory Jr.; his siblings, Obion Perkins of Long Beach, CA, Herbert "Butch" Perkins (Judy) of Matteson, IL, Treston Perkins (Eloise) of Dallas, TX, Ronald Perkins (Christine) of Springfield, IL, Kent Dade (Pamela) of Quincy, IL, Brent Dade (Karen) of Chicago, IL, Don Dade (Shelly) of Ankeny, IA and Diana Osborne of St. Louis, MO.; and his aunts, Beverly Lewis (late Milton) of Quincy and Jackie Harper (George) of Quincy. He is also survived by great-grandchildren.

“Our prayers and sympathy are with you and your family. I met Chuck over 30 years ago when I joined his dojo. He was a great martial arts technician, and his kata form impeccable. Sensei Neil affiliated us with Master Oyata in KC which took our training to a new level. Had some great road trips to KC training. I found Chuck to be a man of integrity, he was humble and had good values. He took care of his family and was fiercely loyal to you Doretta. Chuck will be missed. Bill & Ruth Young”
