Hazelett (Washington, Iowa), born in Ainsworth, Iowa, who passed away on April 29, 2019, at the age of 80, leaving to mourn family and friends. He was predeceased by: his parents, Paul Fredrick and Myrna Carol Hazelett (Sands); and his sisters-in-law, Joan Hazelett and Jan Hazelett. He is survived by: his wife Phyllis Ann Brown; his sons, Brian Hazelett (Tracie) of Raleigh, Brad Hazelett (Dawn Roeder) of St. Paul, Minnesota, Brent Hazelett (Jennifer) of Cary and Greg Hazelett (Kristin) of Muscatine, IA; and his siblings, Paul G. Hazelett (Sue) of Washington, IA and Myrna M. Tappan (Don) of Leesburg. He is also survived by 7 grandchildren.
