He is survived by: his wife Donna Fordham of Washington, Iowa; his daughters, Esther Vazquez of Goshen, Indiana, Sarah Smith (Tony) of Washington, Iowa and Ruth Melvin (John) of Henderson, Texas; his grandchildren, Marcos and Maya Vazquez; his brother Daniel Fordham of Henderson, Texas; his stepchildren, Karla Bell (Scott) of Marina, California, Keith Harper of San Antonio, Texas and Elissa Quinn (Lloyd) of Pittsburg, Texas; and his stepgrandchildren, Matthew, Adam, Hannah, Jacob Bell, Connor, Landon, Tucker and Emersyn Quinn.
In lieu of flowers, memorials have been established for St. Judes Children's Hospital.
