He was predeceased by: his parents, Harold Cook and Delores Cook (Rouse); and his daughter Paula Cook. He is survived by: his wife Eleanor Cook; his children, Linda "ReAnna" DeMers (Gerald), Joanie Mills (Marc) and Martin Cook (Carolyn); his sisters, Reita Cook and Leita Duello (Bob); his grandchildren, Aaron Schantz, Charles Mills, Rachel Chrystal (Adam), Christen Mills, Jana Mahmens (Mitch) and Hayley Lawrence (Braxton); and his great grandchildren, Kalyn, Jevan, Jaron Mills, Briggs Chrystal, Blaire, Beau Mahmens and Saylor Lawrence.
