He was predeceased by: his parents, Louie Earl and Mary Ruth Cook (Alberhasky) of Virginia; and his nephew Robbie Cook. He is survived by: his siblings, Steve Cook (Cindy), Sherrie Falkers (John), Nancy Koerner (Ted), Sue Molitor (Jerry), Mike Cook (Deb), Linda Gradeler (Eric) and Chris Cook (Carmen); and his nieces and nephews, Shane Cook, Ben Cook, Angie Thumann, Jeff Falkers, Brett Young, Amy Lusche, Jack Koerner, Steve Spengler, Jason Spengler, Zach Molitor, Nicole Molitor, Kelly Thomas, Jeremy Cook, Danielle Karch, Melanie Gradeler, Tiffany Gradeler, Caitlin Stein, Seth Cook and Cole Cook.

Memorial donations can be mailed to the funeral home to be passed on to his family to donate to causes that were important to Louie.
