He was loved and cherished by many people including: his parents, Charles Cizek and Albina Cizek (Karkosh); his children, Jacqueline Winchip (Louis), Joel (Connie), Jeffrey Traer (Lynn) and Jo Ellen Kubik (Ray); his sister Doris Formanek; his brother-in-law Raymond Formanek; his sister Margaret Pavelka; his siblings-in-law, Donald Pavelka, Paul Skarda (Mildred Skarda) and Pauline Skarda; his nephew Dr. James Skarda; his close friend Sandy Boomer; his children, Jacque, Jeff and JoEllen; his grandchildren, Angela DeKoster (John), Charlie Cizek (Carrie) and Michael Cizek; and his children, Gina Merrill, Sara Younge (Joe), Liza Pullen (Jeff), Cara Hintz (Brian), Bryant Cizek (Ashley), Tyler Cizek (Ashton), Cassie Schill (Matt), TJ Kubik (Chanlie), Alex Kubik (Lisa) and Sierra Hagedon (Jordan). He was also cherished by great-grandchildren.
