He was predeceased by: his parents, Louis Chmelik and Tillie Chmelik (Berka); his sister June Feisel (Clifford); his nephew Larry Feisel; his brother Kenneth Chmelik; and his siblings-in-law, Walt Shellenberger and Caroline Hanus (Elaine).
He is survived by: his wife Dot Chmelik of Tama; his children, Connie Chmelik of Tama and Allen Chmelik (Tonya) of Tama; his grandchildren, Alex Chmelik, Lydia Chmelik-Barthelmes, Derek Gaydos (Natasha) and Kyle Gaydos (Emily); his great grandchildren, Cole, Braxton, Dalyn, Nylah and Lilly; his sister LaVon Shellenberger of Colorado Springs, CO; and his brother-in-law Keith Hanus of Marshalltown. He is also survived by several nieces and nephews.
