He is survived by: his father Paul Polka of Tama; his wife Teri McFate of Tama; his children, Mathew Polka of Tama, Leah Kearney (Terry) of Springville, IA and Samantha Chapman of Iowa; his siblings, Patty Youngman (Bob) of Tama, Bill Polka (Brendie) of Carlisle and Marty Polka (Lori) of Norwalk; his stepdaughters, Jamie Lewis of Toledo and Rebecca Halverson of Garwin; his grandchildren, TJ, Brooke, Caiden and Killion; and his siblings-in-law, Lena Polka and Dolan Schafer.
