She is survived by: her daughter Jaclyn (Micheal Nelson); her grandchildren, Carli Chacon, Isaac Chacon and Brayden Nelson; her mother Lucille Madsen of Utah; and her siblings, Debbie Brown of Utah, Brenda Whittle of Florida and Jimmy Mac Intyre of Kentucky. She is also survived by nieces, nephews and other relatives.
