He was predeceased by: his parents, Clayton Smith and Marjorie Smith (Jacobs); and his brother-in-law James Osterholt. He is survived by: his former wife Tina Osterholt; his wife Patty Dolphin of Sioux City, IA; his children, Levi Smith (Michelle) and Megan McMann (Mary); his stepson Nicolas Petty; his grandchildren, Kira Petty and Liam Smith; his siblings, Theresa Johnson (John), Sandi Rasmussen (El), Linda Dimond (Richard), Connie Plueger (Rick), David Smith (Lori), Pattie Osterholt, Mike Smith (Sheila), Hulet, Jeffrey Smith (Miriam), Mike Johns (Hollie), Mary Jo Johns (Patrick Dirks) and Mitchell Smith; and his caregivers, Alan, Theresa, Shelly and Dr. Michelle Abrams. He is also survived by his friends and family.
