He was predeceased by: his parents, Charles Cook and Ruth Cook (Baker); his daughter Lori Miller; his granddaughter Elizabeth Chapman; and his siblings, Iris Steckly, June McIntire, Charlene Patton and Charles Cook Jr..

He is survived by: his wife Kathy of Sidney, IA; his children, Becky Chapman of Shenandoah, IA and Shane of West Memphis, AR; and his grandchildren, Kevin Powell, Christopher Miller of Bellevue, Emma Chapman of Sidney, Elise Chapman of Washington, DC and Ethan Chapman of Shenandoah. He is also survived by several great grandchildren, nieces and nephews.
