He was predeceased by: his wife Katy (Neeley). He is survived by: his children, Katy Evenson (Dave), Anne Jacobson (Peter) and John Shelp (Julie). He is also survived by 10 great-grandchildren.
Jay Forrest Shelp will be remembered for his significant contributions to the world of theater, his loving family, and his deep faith.
