He is survived by: his daughter Megan Marie (Joe); his wife Toni Hazelwood of Riverside, Iowa; his siblings, Will Hazelwood (Connie) of Ottumwa, Iowa, Richard Hazelwood (Lennice) of Ottumwa, Iowa, John Hazelwood (Trina) of Ottumwa, Iowa, Karen Johnson of Tennessee, Mary Beth Ott of Boone, Iowa and Carol Storto (Chris) of Ottumwa, Iowa.; and his siblings-in-law, Darrell Chalupa (Denise) of Washington, Iowa, Ronnie Chalupa (Gary Libenguth) and Melody Scott of Grand Forks. He is also survived by many nieces and nephews.
