He was predeceased by: his parents, John "Jack" Thomas Johnson and Genevieve Smith Johnson; his brothers, John, Charles and Theodore; and his nephew Dennis Junior.
He is survived by: his spouse Jackie Tomes; his sons, Chad (Danette) of Des Moines, Eric (Kelly) of Prole and Justin of Churchville; his grandchildren, Breanna Owen (Stetson), Scottie Sarsfield (Liz), Alex Hogue (Brian), Emily, Olivia, Alissa, Jack, Cole, Lucas, Dillon, Maclane and Maddox; his great grandchildren, Jaden, Katelyn, Adelynne, Kellen, Anna, Beau and Lucy; his siblings, Joanna (Bill), Dennis (Judy), Madonna and Dan (Jan); and his pet Doc. He is also survived by many nieces, nephews.
In lieu of flowers, the family asks that memorial contributions be directed to the family for a future memorial.
