He was predeceased by: his parents, Howard Willis Worthington and Ethyle Hodgson Worthington; his children, Ronald, Jeffrey Worthington and Shirley Carter; his siblings, Larry Worthington and Shirley Crane; and his niece Teresa Van Wyk.
He is survived by: his wife Charlotte L. Sams; his son David Worthington (Kathy) of Des Moines; his stepchildren, Janet Bennett of Sardis, TN, Douglas Wallace of Lexington, TN and Jane Parmer (Jim); his son-in-law Bill Carter of Lacona, IA; and his siblings, Max Worthington (Donna) of Newton, IA, Rose Ann Johnson (Dave) of Macon, MO and Craig Worthington (Lannie) of Ames, IA.. He is also survived by ten grandchildren, 4 step grandchildren and many great grandchildren.
