He was predeceased by: his mother Donna Rose Blair (Haeberlein); his mother-in-law Patricia Miller; and his nephew Tye Miller. He is survived by: his wife Heidi Miller Blair; his daughters, Ashley Allen (Jeff) of Fort Dodge and Michelle Blair (Tanner Sporleder) of Altoona; his grandchildren, Abby, Mack and Doss Allen; his brothers, John Blair of Paton and James Blair (Suzanne) of Ellston; his father-in-law Dale Miller; and his siblings-in-law, Holly Shook (Tom), Steve (Linda Aaker), Scott Miller (Jan), Shannon Miller (Beth) and Stacy Miller Harcourt (Jill). He is also survived by 17 nieces and nephews, 11 great-nieces and nephews, and many relatives and friends.
