He is survived by: his wife Cathy Hartz; his sons, Michael Van Gundy (Jenny) of Yale and Craig Van Gundy of Watertown, Wisconsin; his grandchildren, Lindsey, Carley and Lyle; and his sisters, Karen Hawbaker (Rick) of St. George and Kyla Nicholas (Dave) of Panora. He is also survived by many nieces and nephews.
