He was predeceased by: his parents, Arthur J. Slack and Irene A. Slack (Barnett); and his nephew Gregory Slack. He is survived by: his former spouse Daina Dye; his daughters, Amanda Allsup of Oskaloosa and Jessica Slack (Sarah Speranske) of Oskaloosa; his grandchildren, Gavin Meeks, Gauge Allsup, Dace Meeks and Bryant Allsup; and his siblings, Diane Hicks of Ottumwa, Mark Slack (Patty) of Kirkville, Joseph Slack of Ottumwa and Ronald Slack (Cathy) of New Sharon. He is also survived by numerous nieces and nephews.
